DAYCARE, BOARDING, AND TRAINING Policy Agreement

DAYCARE, BOARDING AND TRAINING POLICY AGREEMENT:

• Payments:

Deposits, fees, and/or payments are non-refundable but may be used as a credit in specific situations.
Payment is required 72 hours prior to the session/boarding/care; otherwise, it will be canceled.
Accepted payment methods: Cash, check, or Invoice (Square).
Rescheduling is not guaranteed.

• Cancellation/Rescheduling/Other:

Cancellation or rescheduling must be done at least 72 hours before the scheduled time.
Late cancellations, early pickups, late drop-offs, and "No-Shows" will be charged at the normal rate.

• Late Pickup:

Incurs an additional non-refundable charge of $30 due at the time of pick-up unless agreed with Kate in writing.

• Leash up:

All dogs must remain on a leash during training sessions.
All dogs must wear a martingale for safety purposes.

• Recording of Services:

Photos and videos of training require written permission.
Request permission by emailing kate@kk9privatetrainer.com.

• Hours:

Office hours from 7 am-5 pm/ Operating hours 24/7.
After-hours sessions subject to additional charges, to be discussed beforehand.

• Vaccinations and Health:

Dogs must be up to date on vaccinations (rabies, DHPP, Bordetella).
Dogs must be free from fleas, ticks, and contagious illnesses.
Kate reserves the right to refuse entry or request removal if a dog shows signs of illness.

• Behavior:

Clients must disclose any known behavioral issues or aggression concerns.
Kate may separate or remove any dog exhibiting aggressive behavior for safety.

• Emergency Contact:

Clients must provide an emergency contact name and phone number.

• Liability and Release:

The daycare provider is not liable for injuries or illnesses during the dog's care.
The client releases the daycare provider from liability related to accidents, injuries, or damages caused by the dog.

• Medical Care:

The client is responsible for veterinary expenses during the dog's stay.

• Media Release - Photo and Video Usage Authorization:

Explicit consent to use photographs and videos for promotional and educational purposes.
Authorization is voluntary and may be revoked in writing at any time.

• Termination of Services:

Either party may terminate with written notice. No refunds unless termination is due to a safety issue.

• Right to Decline:

Kate Skiles (Evans) of Kates Kanines has the right to decline or terminate services for any reason.

• General Agreement:

Clients agree to comply with all instructions, rules, and decisions of the daycare provider and this Policy.

• Agreement on Dog's Health:

The client affirms that the dog is free of infectious diseases and has current vaccinations.

• Liability Waiver:

Clients waive, release, and discharge the daycare provider from all claims arising out of or related to daycare services.

• Guaranteed Results:

NO guarantee of results; the owner's commitment and consistency are integral to success.

• Media Release - Photo and Video Usage Authorization:

• By signing this agreement, the client provides explicit consent and authorization to the daycare provider, Kate, to utilize photographs and videos featuring the client's dog(s) for promotional and educational purposes. This permission includes but is not limited to the use of such media on the daycare provider's official website, social media platforms, promotional materials, and any other marketing channels deemed appropriate by the daycare provider.

• The purpose of utilizing these images and videos is to showcase the positive experiences and activities of the dogs under the care of the daycare provider. This may include, but is not limited to, displaying the dogs engaging in play, training sessions, and other activities that reflect the high standard of care provided by the daycare.

• The Client acknowledges that this authorization is voluntary and may be revoked in writing at any time. However, revocation of this authorization does not apply to media content that has already been legally published or used by the daycare provider before the receipt of the written revocation.

• The client affirms that they have read and fully understand the terms of this Media Release and willingly grant permission for the use of their dog's images and videos as described herein.
